Appeal letter
To the claims appeal team,
I am requesting reconsideration of the denial of Maple’s claim. The stated reason was a pre-existing condition. The attached records set out the clinical history relevant to that decision.
The policy wording
Please assess the documented first signs against the definition in the applicable policy, rather than treating an earlier, unrelated visit as evidence of the claimed condition.
The supporting evidence
The dated timeline and veterinary statement distinguish the earlier visit from the symptoms described in this claim. Each referenced record is included with this appeal.
Please confirm receipt and provide the policy clause and clinical evidence relied on in your response.

What Healthy Paws says "pre-existing" means
- Healthy Paws' FAQ defines a pre-existing condition as an illness or condition that "developed or redeveloped prior to the pet policy effective date", plus pre-coverage injuries and their complications.
- Its coverage page says "there doesn't need to be a diagnosis": signs or symptoms before coverage started can be enough.
- It tells customers to see their State Amendatory Statement for their state's definition. For example, the Pennsylvania disclosure uses the narrower wording "signs or symptoms directly related to the condition for which a claim is being made."
The "curable condition" rule
Healthy Paws says certain curable conditions may become eligible if your pet has been free of clinical signs and symptoms and untreated for a continuous 365 days. It lists examples of curable conditions, such as parasitic infections, conjunctivitis, respiratory infections and kennel cough. It gives examples it does not treat as curable, such as cruciate conditions.
An appeal on this ground needs unbroken records covering the 365-day window. A single recurrence restarts the clock.
What we build for a Healthy Paws appeal
- A dated timeline comparing the first clinical sign to your policy effective date, or a 365-day symptom-free count for curable conditions.
- An appeal letter that quotes your state's definition, which may be narrower than the general FAQ wording.
- A pre-filled vet statement on first observed sign, or on resolution date and no recurrence.
- A checklist with Healthy Paws' published channels.
Healthy Paws process notes (check before you send)
- Published contact channels: Customer Care 855-898-8991, help@healthypaws.com. Claims documents: claims@healthypawspetinsurance.com, fax (888) 228-4129, mail PO Box 1156, Portland, ME 04104. If your denial letter gives a different address for appeals, use that one.
- Claim submission has its own time limit — check your policy. Don't confuse it with the appeal deadline.
- Appeal deadline: Appeal deadlines are in your denial letter — check yours.
How it works
- Step 1
Upload in about 5 minutes
Upload your denial letter, policy or declarations page, and available vet records. Phone photos are fine. If something is missing, we tell you what to request from your vet.
- Step 2
We read it against your policy
We identify the stated denial reason and connect your records to the relevant policy definitions. The dates and clinical history become a timeline that addresses the reason your insurer gave.
- Step 3
Finished pack within 1 hour
After your uploads, receive a personalised appeal letter, a pre-filled vet letter to edit and sign, a dated timeline and an evidence checklist. Your pack includes what to attach, how to send it and next steps.
- Step 4
You send it
Use the checklist to bring your attachments together and send the appeal to your insurer. Check the deadline in your denial letter. You stay in control; we never contact your insurer or vet.

The wording matters
Built around your policy wording
Your denial letter identifies the reason. Your policy defines the terms. Your records provide the context. Your appeal pack brings those pieces together.
The appeal letter connects the stated denial reason to the applicable policy wording and the supporting records, with a dated timeline and a pre-filled vet letter.
Example only. Maple and the case details are fictional. Your pack is prepared from your own uploaded documents.
