Appeal letter
To the claims appeal team,
I am requesting reconsideration of the denial of Maple’s claim. The stated reason was a pre-existing condition. The attached records set out the clinical history relevant to that decision.
The policy wording
Please assess the documented first signs against the definition in the applicable policy, rather than treating an earlier, unrelated visit as evidence of the claimed condition.
The supporting evidence
The dated timeline and veterinary statement distinguish the earlier visit from the symptoms described in this claim. Each referenced record is included with this appeal.
Please confirm receipt and provide the policy clause and clinical evidence relied on in your response.

What "pre-existing" usually means on a Nationwide policy
- Nationwide pet policies generally exclude conditions that first showed signs, were diagnosed or were treated before coverage started, or during a waiting period. The definition that applies to you is the one in your policy document.
- Several plan types: Nationwide has sold different pet plans over the years, including whole-pet and accident/illness plans, and some are offered through employers. Exclusions and waiting periods can differ between them, so check which plan you have.
- Curable conditions: some policies let a condition that has resolved be covered again after a symptom-free period. Whether yours does, and for how long, is set out in your policy wording.
- Waiting periods: we use the waiting periods stated in your policy, not general figures.
Where Nationwide appeals usually focus
- Same condition or a different one: whether a pre-policy note really describes the condition you claimed for.
- First-sign date: when the vet records show the first sign, compared with your start date and waiting period.
- Resolution: where your policy allows it, records showing an earlier issue resolved and stayed resolved.
- Plan wording: whether the reason given matches the exclusion actually written in your plan.
What we build for a Nationwide appeal
- A timeline marked with your policy start date, the end of each waiting period, and every relevant vet visit.
- An appeal letter quoting the definition in your Nationwide policy document.
- A pre-filled vet statement matched to the denial reason: first sign, resolution, or a different condition.
- A checklist of what to send, how to send it, and what to keep.
Nationwide process notes (check before you send)
- Where to send it: use the contact details and appeal instructions in your denial letter or your Nationwide pet account. Ask in writing if anything is unclear.
- Keep proof: send in a way that shows delivery, and keep copies of everything you send.
- Appeal deadline: Appeal deadlines are in your denial letter — check yours, and check your policy for the claim submission limit too.
How it works
- Step 1
Upload in about 5 minutes
Upload your denial letter, policy or declarations page, and available vet records. Phone photos are fine. If something is missing, we tell you what to request from your vet.
- Step 2
We read it against your policy
We identify the stated denial reason and connect your records to the relevant policy definitions. The dates and clinical history become a timeline that addresses the reason your insurer gave.
- Step 3
Finished pack within 1 hour
After your uploads, receive a personalised appeal letter, a pre-filled vet letter to edit and sign, a dated timeline and an evidence checklist. Your pack includes what to attach, how to send it and next steps.
- Step 4
You send it
Use the checklist to bring your attachments together and send the appeal to your insurer. Check the deadline in your denial letter. You stay in control; we never contact your insurer or vet.

The wording matters
Built around your policy wording
Your denial letter identifies the reason. Your policy defines the terms. Your records provide the context. Your appeal pack brings those pieces together.
The appeal letter connects the stated denial reason to the applicable policy wording and the supporting records, with a dated timeline and a pre-filled vet letter.
Example only. Maple and the case details are fictional. Your pack is prepared from your own uploaded documents.
