Appeal letter
To the claims appeal team,
I am requesting reconsideration of the denial of Maple’s claim. The stated reason was a pre-existing condition. The attached records set out the clinical history relevant to that decision.
The policy wording
Please assess the documented first signs against the definition in the applicable policy, rather than treating an earlier, unrelated visit as evidence of the claimed condition.
The supporting evidence
The dated timeline and veterinary statement distinguish the earlier visit from the symptoms described in this claim. Each referenced record is included with this appeal.
Please confirm receipt and provide the policy clause and clinical evidence relied on in your response.

What Trupanion says "pre-existing" means
In its public FAQ, Trupanion describes a pre-existing condition as an illness, condition or injury with "signs or evidence of their potential manifestation" within the 18 months before your policy's effective date (or during waiting periods). It also includes injuries, or recurrences of injuries, that happened before coverage.
At enrolment, Trupanion says it requests your pet's past medical records and a claims specialist assesses them.
Trupanion's FAQ also gives an example: a later condition was covered because specialists found a "medical disconnect" between it and an older injury. That idea, a medical disconnect, is often the heart of a pre-existing appeal.
Your own policy wording may differ by version and state. We quote your document, not this page.
Why Trupanion pre-existing denials often come down to one chart note
"Signs or evidence of potential manifestation" is broad wording. A pre-policy note can be read as an early sign of a later illness. Examples: an owner remark, an incidental finding, or a "monitor" comment. A strong appeal shows, with dates and your vet's opinion, whether that note is medically connected to the condition you claimed for.
What we build for a Trupanion appeal
- A timeline of every pre-enrolment note Trupanion may have seen, set against the date of first clinical sign for the claimed condition.
- An appeal letter that quotes your policy's pre-existing clause and addresses each record Trupanion cited.
- A pre-filled vet statement on whether the earlier note relates to the claimed condition. Your vet edits and signs.
- A checklist that includes any appeal form step Trupanion asks for.
Trupanion process notes (check before you send)
- Appeal form with your vet: Trupanion's published complaint protocol encourages owners to work with their veterinarian to complete an appeal form and attach records. The exact US process may differ — check your denial letter or ask Trupanion.
- Appeal deadline: Appeal deadlines are in your denial letter — check yours. If it isn't stated, ask Trupanion and note the answer.
- Claim submission window (different from the appeal window): check Trupanion's claims page and your policy version for the current limit.
- Where to send it: use the contact shown on your denial letter or in your Trupanion account.
How it works
- Step 1
Upload in about 5 minutes
Upload your denial letter, policy or declarations page, and available vet records. Phone photos are fine. If something is missing, we tell you what to request from your vet.
- Step 2
We read it against your policy
We identify the stated denial reason and connect your records to the relevant policy definitions. The dates and clinical history become a timeline that addresses the reason your insurer gave.
- Step 3
Finished pack within 1 hour
After your uploads, receive a personalised appeal letter, a pre-filled vet letter to edit and sign, a dated timeline and an evidence checklist. Your pack includes what to attach, how to send it and next steps.
- Step 4
You send it
Use the checklist to bring your attachments together and send the appeal to your insurer. Check the deadline in your denial letter. You stay in control; we never contact your insurer or vet.

The wording matters
Built around your policy wording
Your denial letter identifies the reason. Your policy defines the terms. Your records provide the context. Your appeal pack brings those pieces together.
The appeal letter connects the stated denial reason to the applicable policy wording and the supporting records, with a dated timeline and a pre-filled vet letter.
Example only. Maple and the case details are fictional. Your pack is prepared from your own uploaded documents.
